Diagnosed with alopecia areata, prurigo nodularis or atopic dermatitis
US Resident
11-17 years old (patient)
18+ (caregiver)
Please note: Parent/guardian/caregiver must complete the screener on behalf of the child/adolescent if under the age of 18 and be present for the interview. Demographic questions will be asked about both the parent/guardian and the child during this screener.
